Connections for Cardiovascular HealthSM Announces Grant to West Virginia Health Right, Inc.

The AstraZeneca HealthCare Foundation program, Connections for Cardiovascular HealthSM, today announced a grant of more than $185,000 to the West Virginia Health Right, Inc. in Charleston in support of the Pathways to Cardiovascular Health program.

Pathways to Cardiovascular Health is designed to improve the health of 500 impoverished uninsured patients through clinical intervention. The majority of the clinic's impoverished, uninsured patients have one or more cardiovascular high risk factors.

“We are honored that the AstraZeneca HealthCare Foundation shares our mission and has chosen to support our efforts to improve the cardiovascular health of our patients,” said Patricia H. White, Executive Director of WV Health Right. “We witness the debilitating effects of cardiovascular disease on a daily basis and have long recognized that this chronic illness requires a combination of coaching, health education, self-management and family involvement as well as medication therapies. Further, we applaud the Foundation’s commitment to funding programs that seek to prevent cardiovascular disease in a vulnerable and medically underserved population: the impoverished uninsured.”

Pathways to Cardiovascular Health is designed to improve the health status of low-income uninsured adults through participation in health education classes, screenings and disease self-management. Existing successful smoking cessation, weight loss, and diabetes management programs will be expanded to accommodate additional patients.

“We must work to decrease the risk of and prevent cardiovascular disease in the United States, by providing resources to those in need,” said Emily Denney, Trustee, AstraZeneca HealthCare Foundation. “West Virginia Health Right is providing an innovative program to improve cardiovascular health in communities across West Virginia.”

Connections for Cardiovascular Health was launched in 2010 through a charitable contribution of $25 million from AstraZeneca. The program awards grants of $150,000 and up to US-based non-profit organizations that are doing innovative work in the field of cardiovascular health. The grants will be used to focus on screening initiatives that will identify patients with or at-risk of cardiovascular disease. Those individuals who do not have health insurance or are underinsured will be given the option of establishing a medical home with the free health clinic.
