These excerpts taken from the GWW 10-K filed Feb 27, 2008. Claim for Benefits. Any claim for benefits under the Plan shall be made in writing to any member of the Committee. If such claim for benefits is wholly or partially denied by the Committee Members, the Committee Members shall, within a reasonable period of time, but not later than sixty (60) days after receipt of the claim, notify the claimant of the denial of the claim. Such notice of denial shall be in writing and shall contain:
(a) the specific reason or reasons for denial of the claim,
(b) a reference to the relevant Plan provisions upon which the denial is based,
(c) a description of any additional material or information necessary for the claimant to perfect the claim, together with an explanation of why such material or information is necessary, and
(d) an explanation of the Plans claim review procedure.
