When consumers enroll for 2016 health coverage via exchanges under the Affordable Care Act, they are less likely to find a preferred provider organization (PPO) plan, which could limit their doctor and hospital choices.
(MedPage Today) -- The cost of healthcare for a typical family of four enrolled in a preferred provider organization has more than doubled since 2002 -- from $9,235 to $19,393, according to the 2011 Milliman Medical Index (MMI).
